Enter the OTP and click on ‘Submit’. The website will then verify your eligibility for the Ayushman Bharat scheme. If you are eligible, you will be directed to a new page.

Step 3: Enter your personal details

On the new page, you will be required to provide your personal details such as name, age, gender, address, etc. Fill in all the necessary details and click on ‘Submit’.

Step 4: Upload supporting documents

After submitting your personal details, you will be required to upload certain supporting documents.

These may include your Aadhaar card, ration card, or any other document that verifies your identity and eligibility for the scheme.

Step 5: Submit your application

Once you have uploaded all the necessary documents, review the information you have provided and click on ‘Submit’.

Your application for an Ayushman card will be processed, and you will receive a confirmation message on your registered mobile number.
